An electronic health record, patient, workforce and business management solution that enables healthcare organizations to improve patient care while creating clinical and business efficiencies.
Depending on business need, Concordia is delivered over the internet to a wide range of devices—from PCs, laptops and tablets, to PDAs and mobile phones—to provide information at the point of care for mobile community-based workforces.
The Concordia point of care solutions, available as standalone or integrated, include:
- Mobile Wound Care: a specialized point of care solution that enables communication across the continuum of care for patient wound care management.
- InterRAI: a point-of-care application that automates the manual steps of the InterRAI screening process, including reporting. It is also available for mobile devices. (PDF)
A "plug and play" connection service and toolkit that enables secure, standards-based multi-party exchange of information between Concordia and the various participants in the healthcare ecosystem including labs, pharmacy, decision support and other clinical systems.
A flexible component of the Healthphone platform enabling interactive, SMS/MMS-based content delivery and reinforcement to consumers in support of preventive care and wellness programs. These programs include smoking cessation, exercise, nutrition, medication reminders and weight management.
The following pre-configured HME applications are available today:
- STOMP (Stop Smoking by Mobile Phone): a smoking intervention application delivered to participants via their mobile phones. It supports individuals in overcoming nicotine addiction by enabling them to be true stakeholders in their own recovery through text messages that offer quit tips and craving distractions.
