RADT (Referral, Admit, Discharge Transfer)

• Referral - Sending a patient from one practitioner to another for healthcare services, including pre-admission and waitlists.
• Admit –The final step in the Concordia intake workflow which takes place on the date that the patient is admitted to the service/programme.
• Transfer - Includes all the steps required to temporarily transfer the care of a patient from their current organization to another organization/care provider, i.e. a family member or the patient themselves.  The transfer is temporary and is associated with a planned return date.
• Discharge - Patients are discharged upon meeting their referral goals.
