Closing the gap healthcare
Closing the Gap Healthcare Group is an award-winning Canadian business that provides community-based rehabilitation therapy, nursing, and personal support services of an exceptionally high quality. Their nearly 700 staff members provide services in homes, schools, long-term care homes, hospitals, and clinics.
Health providers use a mobile device, such as a laptop or Personal Digital Assistant (PDA), at the point of care to view and update patient records, access test results and instantly share information with other health providers.
Closing the Gap has pioneered the use of Community Care Management Solution (Concordia is marketed in Canada by TELUS as CCMS) that assists them to optimize direct patient care time, increase patient safety through one-time recording of clinical notes at the point of care, and improve patient outcomes by facilitating sharing of healthcare information across the continuum of care.
Canadian telco partner TELUS implemented the CCMS solution, hosting and managing the application, providing network connectivity and security support, 24x7 customer service and required hardware such as PDA’s and mobile phones, as well as all infrastructure and servers. The CCMS application is delivered as a fully hosted and managed service using the Microsoft Office platform.

The project is designed to track and measure effectiveness in wound care treatment and enhance access to scarce specialized resources.
